COVID19 Screening Guidelines

Due to the recent COVID19 pandemic, all dental offices are required to screen any person(s) entering their building for possible exposure and/or symptoms of COVID19.
 
We will ask you these questions:

  • Two (2) days prior to your child(ren)'s appointment. This will be sent as a text message. If your child has been ill with COVID-like symptoms or has had a direct exposure to COVID, the appointment should be rescheduled.
  • The day of your child(ren)'s appointment, when you call and check in from your car.
 
Please note that these screening questions will be asked for both your child(ren) AND the caregiver that will be accompanying them to their dental appointment.
 
Please take time to review the questions now and contact our office if you answer "yes" to any of the questions regarding your child(ren) and/or the accompanying caregiver.

Do you, your child, or anyone you’ve been in close contact with (ex. siblings, extended family, daycare) have ANY of the following symptoms?

 

  1.  Fever

  2.  Respiratory symptoms (cough, shortness of breath, sore throat)

  3.  Flu-like symptoms (headaches, fatigue, weakness)

  4.  GI upset

  5.  Recent loss of taste and/or smell

  6.  History of travel outside of Iowa within the last 14 days

  7.  Have been in contact with someone who:

    1.  has tested positive for COVID19 or

    2.  is under investigation for possible COVID19 infection or

    3.  has any other illnesses that could be transmitted

 

If yes, please know we must reschedule your appointment, per recommendations and guidelines from the CDC.

 

(If an existing patient has a dental emergency and current COVID19 symptoms, contact our office and we will make appropriate accommodations on a case-by-case basis and based on the severity of the emergency.)
